A Clinical Study of Mechanical Obstruction due to Postoperative Adhesion

Abstract
|
|
|
This is a clinical analysis and review of 114 cases of intestinal obstruction caused by postoperative adhesion which were treated at this department of Chungnam National University Hospital during 7 years from March, 1975 to February, 1982.
1) The majority of patients ranged 30 to 59 years of age were 66 cases(57.9%) and male to female sex ratio was 1.7:1.
2) The common causes of the postoperative adhesive ileus were appendectomies in 30 cases (26.3%), operations on stomach and duodenum in 27 cases (23.7%) and gynecologic operations in 21 cases(18.4%).
3) The duration between previous operations and postoperative ileus were as follows. Twenty-five cases(21. 9%) developed within a month and 63 cases (55.3%) within a year after the previous operation.
4) As for the method of treatment: Fifty-two cases (45.6%) were treated conservatively. Surgical treatment was attempted for 62 cases, including adhesiolysis of 38 cases(33.3%), intestinal resection of 16 cases(14%) and intestinal plication of 5 cases(4.4%).
5) The incidence of postoperative complication was 17 cases (27.4%) and the most common of them was wound infection in 8 cases(l2.9%).
6) The overall mortality was 5 cases(4.4%), but that of strangulated intestinal obstruction was 4 cases(25%).
